Service Description

Provides clinical and educational resources to BC youth ages 12 to 18, their families, and health care providers. Counsels youth about personal, social, and physical development, mental well-being, substance use, sexual health, safety, and risk-taking behaviors, as well as transition to adulthood and possibly adult care in relation to health. Serves youth with chronic medical illness or a disability, complex undiagnosed condition, personal or family crisis, risky health behaviours, sexual health questions or concerns, or medical treatment or pediatric transition planning needs. Also offers On-Trac materials that support youth-focused care and transition planning for youth moving from pediatric to adult care. Referrals required from service providers.
