CARES (Community Action and Resources Empowering Seniors) Social Prescribing for Seniors

Service Description

Connects adults ages 65 and over who require support with non-clinical community services to help them address their health and well-being needs. Serves older adults who are experiencing decline in physical and/or emotional health, feel isolated, make frequent trips to the emergency room or primary care, require support in order to remain independent, want to become more physically active and/or socially connected, and/or are experiencing language and cultural barriers. Social prescription works alongside clinical treatments to address people's needs in a holistic way, and may include personal coaching, emotional support, and information and education in the areas of physical activity, social connection, and nutrition/food security. Referral required from a physician or other health care practitioner. Language support through interpretation is available. Services are offered by phone, email, video calls, or in person by appointment. Office hours are 9 am to 5 pm Monday to Friday. Funded by Ministry of Health (MOH) and managed by United Way BC.
