Czorny Alzheimer Centre

Provides 72 publicly subsidized, long-term care beds in six secure, home-like cottage living spaces for eligible individuals with Alzheimer's disease; all rooms are single occupancy. Intake is via Home Health Service Line; do not contact the facility directly. Serves Abbotsford, Agassiz, Anmore, Belcarra, Boston Bar, Burnaby, Chilliwack, Coquitlam, Delta, Harrison Hot Springs, Hope, Kent, Langley, Maple Ridge, Mission, New Westminster, Pitt Meadows, Port Coquitlam, Port Moody, Surrey, and White Rock. Office hours are 8 am to 4 pm Monday to Friday.
