Fall Prevention Program

Service Description

Provides services to Richmond residents age 65 and over and their families. Services are offered in the home and/or in the clinic that includes a falls risk assessment (including identification of fall hazards in the home, and strength and balance status), recommendations for reducing the risk of falls, and referral to other health professionals or community services. Interdisciplinary team consists of a public health nurse, physiotherapist, and occupational therapist. Clients must be medically stable, cognitively intact, able to direct their own care (or have a competent caregiver), and able to understand the program. Referral required; a referral form is available online. Service is provided in English; interpreter services are available. Also provides education and resources for community groups, and groups working with seniors. Office hours are 8:30 am to 4:45 pm Tuesday to Thursday.
