Falls Prevention Mobile Clinic

Service Description

Offers a personalized assessment for older adults living in the community who have a history of falls or issues with balance and mobility. Seniors living in residential care homes are not eligible. Seniors with advanced cognitive impairment or those who are not ambulatory may not be suitable for the clinic. Participants meet one-on-one with health professionals (pharmacist, kinesiologist, and physiotherapist) to discuss their risk status and interventions. Provides a detailed report of recommendations to the participant and their primary care physician. Also offers education sessions in the community to discuss fall risk factors, fall prevention, home safety equipment, and fall prevention exercises; see website for a presentation schedule. The clinic travels to many different locations in the Fraser Health region including community centres, seniors housing complexes, and other locations; see website for clinic schedule. Preregistration is required. A referral form is available online.
