Service Description

Fraserdale Long Stay Program is an assisted living program staffed 24-hours for seven adults age 19 and older with mental health challenges to maintain an optimal level of independence for up to 18 months. Also offers medication services and psychosocial supports. Individuals must be willing to participate in an individual recovery plan, have a case manager, be receiving Persons with Disabilities (PWD) benefits, be medically and psychiatrically stable, not have current drug or alcohol issues, and not demonstrate the likelihood of violent or aggressive behavior. Referral must be submitted by the Burnaby SHARP Coordinator or Fraserdale Program Supervisor. Fraserdale Short Stay Program offers temporary supportive housing for mental health clients age 19 and older who are being discharged from a hospital or community care facility and require interim shelter until their own permanent accommodation is available. Individuals must have a case manager who has established an individual housing plan, be medically and psychiatrically stable, not have current drug/alcohol issues, and not demonstrate the likelihood of violent or aggressive behaviour. Referral required through a mental health case manager, other short stay shelter, Royal Columbian Hospital, Burnaby In-Patient Unit (IPU), Community Residential Emergency Short Stay and Treatment Units, or social worker. Respite care may be available for families who care for a mentally ill relative at home, depending on bed space. Registered with the Assisted Living Registry, Ministry of Health.
