Health Promotion Case Management (HPCM) Program

Service Description

Works one-to-one with HIV-negative people who want support to reduce or navigate HIV risk stemming from intergenerational, structural, interpersonal, and individual factors. Case managers work with each client to set and complete goals related to HIV prevention. Program serves people of all identities and offers specialized expertise in working with Indigenous people, queer, trans and nonbinary people, sex workers, folks using substances, and people in the Downtown Eastside. Youth may be eligible on a case-by-case basis. Services include support to access community supports, safe supply, and primary care; HIV testing and medications; navigating access to the Medical Services Plan (MSP) and other government programs; and skill building to talk about sex and set boundaries around substance use. Length of program is typically six to nine months. Open referral; applications are on the website. Hours are 10 am to 4 pm Monday to Thursday.
