Service Description
Provides primary care for North Shore residents with high needs, defined as individuals who don't have a regular family doctor or nurse practitioner and face complex medical, mental health and substance use, and/or socio-economic needs such as housing, income, and access to food. Open referral. Drop in hours are from 9 am to 12 noon Monday to Friday and appointments from 1 pm to 4 pm Monday to Friday. Offered in partnership with North Shore Division of Family Practice.
