Indigenous Patient Navigator (IPN) - Lower Mainland

Provides cultural support to Indigenous patients in acute care and their families, as well as consultation on health care needs. Also provides consultation with medical staff on care planning, and information on Indigenous-specific health resources and referrals. Open referral, including self-referral; application forms are available online and must be completed prior to accessing support. Serves Abbotsford, Agassiz, Anmore, Belcarra, Bowen Island, Burnaby, Chilliwack, Coquitlam, Delta, Deroche, Dewdney, Harrison Hot Springs, Hope, Langley, Lions Bay, Maple Ridge, Mission, New Westminster, North Vancouver, Pitt Meadows, Port Coquitlam, Port Moody, Richmond, Surrey, Vancouver, West Vancouver, and White Rock. Office hours are 8 am to 8 pm Monday to Friday, and 9 am to 5 pm weekends and holidays.
