LinkAGE Social Prescribing

Connects adults ages 55 and over with local, non-medical community programs that support participant health and wellness. Serves older adults living in the West End, Coal Harbour, or Yaletown neighbourhoods of Vancouver, who are at risk of frailty, experiencing social isolation and/or loneliness, making frequent visits to primary care or the emergency room, and/or are in need of support to access community-based services. Services may be in areas such as social and recreational programs, supports for aging in place, mental health services, and/or nutrition/food security. Referral required from physician or other health care provider. Funded by Ministry of Health (MOH) and managed by United Way BC.
