Service Description
Provides assessment and referral services to supported housing for homeless and precariously housed individuals with mental health, substance use, and behavioural issues. Housing varies from supported apartment programs to 24-hour staffed licensed community homes. Accepts referrals from physicians, social workers, community mental health teams, and acute and tertiary mental health sites within Vancouver.
