Service Description

Provides outpatient geriatric services to older adults age 65 and over who are facing changes to their health and well-being, including issues such as complex medical conditions, psychiatric illnesses, dementia, memory and cognition, mobility and balance, medication optimization, depression, future planning, nutrition, and continence. Services include assessment, consultation liaison, and follow-up care through outpatient clinics, inpatient psychiatry, tertiary care, residential care, and outreach. Services include outpatient geriatric psychiatric services for patients with dementia and psychiatric illness such as mood, anxiety, or psychotic disorders. Also provides a Dementia Caregiver Resilience Clinic designed for caregivers of people with dementia. The clinic supports caregiver skill-building, education, and strengthening of self-care strategies through evidence-based group interventions and/or individual sessions with interdisciplinary professionals. Referral for geriatrician or geriatric psychiatrist accepted from family or hospital-based doctors; referrals to other members of the team accepted from any health provider. Hours are 8:30 am to 4:30 pm Monday to Friday.
