Prescribing Wellness for Older Adults

Service Description

Connects adults ages 55 and over who require support to non-clinical multilingual and culturally appropriate social and community services so that participants can play an active role in the management of their own well-being. Action plan enables participants to gain access to the programs that meet their specific needs, including services in the areas of social engagement, physical activity, and nutrition. Prioritizes referrals from doctors and health care professionals, but also welcomes referrals from community services and self-referral. Serves individuals residing in the Centre North Vancouver area, which covers the Downtown Eastside, Grandview Woodlands, and Northeast False Creek neighbourhoods. Funded by Ministry of Health (MOH) and managed by United Way BC.
