Service Description

Provides pre-release supports and services to incarcerated individuals as they prepare for their transition to the community. Reintegration workers assist with pre-release planning and support individuals to identify and connect with services that meet their individual needs and goals. Also provides information on the rights and responsibilities of incarcerated persons, assists with establishing community supports, and works to reduce the number of barriers faced by individuals before their release. Community Support Initiative (CSI) provides personalized support for people with complex needs as they transition from prison to the community. Services include assistance locating and maintaning housing, filing taxes, applying for identification, job placement and retention, and referrals to substance use treatment, mental health, or health services.
