Renfrew Collingwood Social Prescribing Program

Service Description
Connects adults ages 55 and over to local, non-clinical community services that will help them better manage their health and well-being. The social prescription addresses a person's social needs as part of their overall health and wellness plan, and allows for a comprehensive approach to multiple medical issues. Referral required from physician or other health care practitioner. Funded by Ministry of Health (MOH) and managed by United Way BC.
