Seniors Social Prescribing Program

Service Description

Connects Abbotsford older adults to local, non-clinical services so they can better manage their own health and well-being. Serves individuals who may be socially or emotionally vulnerable, physically frail, regularly attending the emergency room, physically inactive, and/or experiencing overall poor health outcomes associated with social determinants of health such as low income. Clients are referred to support services within Archway or elsewhere in the community, including physical activity, skills training, nutrition/food programs, social programs, mental health services, affordable housing, and medical transportation. The program's mission is to delay frailty, encourage self-care management, and reduce loss of independence. Referral required from physician or other health care practitioner. Office hours are 8:30 am to 4 pm Monday to Friday. Funded by Ministry of Health (MOH) and managed by United Way BC.
