Connects adults aged 65 and over (exceptions may apply) with a wide-range of local, non-medical community programs that support participant health and wellness. Serves older adults who are at risk of frailty, experiencing social isolation and/or loneliness, making frequent visits to primary care or the emergency room, low to moderate income, and/or are in need of support to access community-based services. Services may be in areas such as social and recreational programs, physical activity and exercise programs, supports for aging in place (housekeeping, yard maintenance, grocery shopping), affordable housing, medical transportation, family and caregiver support groups, counselling and, meal delivery and/or nutrition/food security. Referral accepted from self or other. Serves Abbotsford. Office hours are 8 am to 4 pm Monday to Friday. Funded by Ministry of Health (MOH) and managed by United Way BC.
