Seniors Social Prescribing Program

Wellness coordinator connects adults age 50 and over to local, non-clinical community services to elevate their quality of life through enhanced social connectedness and physical activity. The prescription may include physical activities, social activities, nutrition/food security programs, caregiver programs, and opportunities to access services in the community. Referral from a medical professional required, although may accept other referrals as well. Offered in collaboration with North East Van Active Aging Network (NEVAAN). Funded by Ministry of Health (MOH) and managed by United Way BC.
