Service Description

Connects adults ages 55 and over to local, non-clinical community services that support participant health and wellness. Serves older adults with higher needs such as frailty, health challenges, loss, and isolation. Action plan enables participants to play an active role in the management of their own well-being by gaining access to the programs and services that meet their specific needs, including in the areas of social engagement, physical activity, and nutrition. Referral required from a physician. Funded by Ministry of Health (MOH) and managed by United Way BC.
