Specialized Seniors Clinic - Chilliwack

Provides specialized health care services to seniors age 65 or older with multiple health issues who are at risk of losing the ability to function independently. Health issues may include chronic disease (dementia, depression, etc), risk of falls or history of falls, difficulty managing medications, changes in the ability to live independently, or caregiver challenges. Interdisciplinary staff provides comprehensive screening, assessment, treatment, care planning, and education to seniors and their families, as well as mentoring support to professional caregivers. A referral is required by a health care professional. Serves Agassiz, Chilliwack, Harrison, and Hope. Hours are 8:30 am to 4:30 pm Wednesdays and Thursdays.
