Sunshine Coast Home Care Services and Seniors Information

Provides home nursing care and rehabilitation services to seniors, people with disabilities, and convalescent individuals; access to adult day care and residential care; and information, education, and support for caregivers. Home Care Teams provide case management and home care nursing services. Home Support provides a range of support options designed to help people with ongoing health conditions to remain in their homes and to be as independent and safe as possible. The Palliative Care Program provides clinical care and support to people with a terminal illness. Choice in Support for Independent Living (CSIL) provides alternative methods for delivering home support for clients who are able to direct their own care. A community dietician is available for consultation, assessment, and individual nutrition planning/training to improve and/or maintain clients' health; open referral. Office hours are 8:30 am to 4:30 pm Monday to Friday.
