Youth in Transition

Service Description

Enables North Shore youth ages 15 to 18 years without a supportive family environment to develop the skills needed to achieve full independence; this includes youth with Continuing Custody status and those receiving service through Youth Agreements. Also provides outreach and ongoing support to high-risk youth ages 16 to 19 who are unable to live with their parents, in a foster home, or in group placement. Services include integrated service planning; life, social, vocation, and employment skill development; youth care interventions; liaison to health and education services; housing support and assistance; after-hours crisis response; referrals to community services including recreational programs; access to support such as parenting programs for youth, and transitional services to support a post-service plan. Training and education in family conflict and conflict resolution, anger management, communication, substance use, medical issues, and budgeting is also provided. Services are available from 9 am to 9 pm Monday to Friday, and from 9 am to 6 pm on Saturdays; support is available for up to one year. Referral and funding is by Ministry of Children and Family Development (MCFD).
