Offers access to preventative and primary care services for patients with complex medical and social needs, mental health and addictions challenges, and coordination of specialist care. Services include chronic disease management; mental health assessment, treatment, and support; substance use treatment and support. Only serves clients who have gone through intake and are attached to the centre; prospective patients must register through the Health Connect Registry and wait to be contacted by the centre. Serves residents of the Oceanside communities of Parksville, Arrowsmith Coombs Country, Bowser, French Creek, Nanoose Bay, Lighthouse Country, and Qualicum Beach. Phone and virtual video appointments with a nurse practitioner are available 8 am to 4 pm Monday to Saturday. In-person and same-day, team-based primary care appointments are available when necessary. Nonprofit society.
