Adult Mental Health Program - Vancouver (Raven Song Community Health Centre)

Multidisciplinary team provides individual and group mental health rehabilitation services to adults with serious and persistent mental illness. Services include psychiatric assessment and treatment, individual and group therapy, and rehabilitation services. Rehabilitation involves skill development, coaching, education, and mentoring in areas such as home management, community living, interpersonal relationships, illness and self-management, volunteering, returning to school, work, recreation, and leisure. Services may involve the client's significant other/family if appropriate. Referral required through Access and Assessment Centre (AAC), physician, psychiatrist, or nurse practitioner. Serves existing clients of Vancouver Community Mental Health Services. Transgender-friendly service. Service may be available in Cantonese and Spanish, depending on staff availability. Free interpretation service is available for various languages. Access and Assessment Centre phone line hours are from 7:30 am to 11 pm daily. Office hours are 8:30 am to 5 pm Monday to Friday.
