Assertive Community Treatment (ACT) - Langley

Provides intensive, individualized outreach mental health services for adults with serious and persistent mental illness (may include coexisting substance use disorder), and significantly impaired functioning, such that they may not have connected with or responded well to traditional outpatient mental health care services. Services include assessment and intervention, psychiatric/psychological treatment, medication management, rehabilitation, supports for substance use disorder, daily living activities, life skills training, and peer support. Program works closely with the individual and their family or other supports to create a plan to improve quality of life and decrease time spent in hospital or with emergency services. Delivered in an ongoing rather than time-limited way by a multidisciplinary team as outreach and onsite services. Referral required through health care provider. Office hours are 8:30 am to 4:30 pm Monday to Friday. Program hours are 8:30 am to 8:30 pm Monday to Friday and 10 am to 6 pm Saturdays and Sundays.
