Assertive Community Treatment (ACT) - Langley

Service Description

Provides intensive, individualized outreach mental health services for adults with serious and persistent mental illness and/or substance use and/or significantly impaired functioning, such that they may not have connected with or responded well to traditional outpatient mental health care services. Services include crisis assessment and intervention, psychiatric/psychological treatment, medication management, rehabilitation, daily living activities, life skills training, peer support, and supports for individuals who use substances. Program works closely with the individual and their family or other support network to create a plan to improve quality of life and decrease contact with hospital/emergency services. Services are ongoing delivered by a multidisciplinary team. Referral required from a hospital or through the justice system. Outreach hours are year-round from 8:30 am to 4:30 pm Monday to Friday, and 10 am to 6 pm Saturdays and Sundays.
