Provides housing and support to adults and seniors with an acquired brain injury, mental health diagnoses, and physical, intellectual, and medical health care needs. The individual shares a home with a person and/or family who is contracted to provide ongoing support. Referral required from the local Home Health Office, Fraser Health. Serves Metro Vancouver, the Fraser Valley, the Sunshine Coast, and Kelowna.
