Connects adults ages 55 and over to local non-medical community supports and services that will help them better manage their health and well-being. Addresses clients' social needs as part of their overall health and wellness plan and allows for a comprehensive approach to multiple medical issues. Referral accepted from self or others. Funded by Ministry of Health (MOH) and managed by United Way BC as a Social Prescribing program.
