Supports older adults by connecting them with non-medical community services. Services include assessment and triaging, co-creation of a wellness plan, follow-up and check-ins, and health system navigation. Referral accepted from self/health care provider/community agency/other. No fee for service. Serves Kaslo. Funded by Ministry of Health (MOH) and managed by United Way BC.
