Works with socially isolated older adults 65 years of age and over who live in the Mount Pleasant, Kensington, Riley Park/South Cambie area of Vancouver to connect with community support and seniors' services. This is a non-medical and non-clinical way of referral to meet older adults' need for social and community support. The program aims to increase quality of life, reduce social isolation, and improve physical and mental wellness for older adults while living independently with respect and dignity. Accepts referrals from Vancouver Coastal Health, community organizations, family or friends, and self-referral. Funded by BC Ministry of Health and managed by United Way BC's Healthy Aging programs.
