Community Connector - Vancouver (West End)

Connects adults 55 and over to local, non-clinical community services that will help them better manage their health and well-being. The Community Connector addresses a person's social needs as part of their overall health and wellness plan, and allows for a comprehensive approach to multiple non-medical issues. Serves West End, Coal Harbour, and Yaletown. Funded by Ministry of Health (MOH) and managed by United Way BC as a Social Prescribing program.
