Offers falls risk assessment, comprehensive geriatric assessment, and individualized recommendations for falls prevention to adults ages 65 and over. Eligible individuals are community-dwelling (including assisted living facilities); have had at least one fall in the previous 12 months; have not been diagnosed with Alzheimer's disease, dementia, or any progressive neurological conditions; and are able to walk at least 10 metres (with or without an assistive device). Service is provided in English; interpreter services are available. Referral is required from a family physician; a referral form is available online. Hours are 9 am to 5 pm Monday to Friday.
