Provides free specialized support for individuals diagnosed with HIV or hepatitis C (HCV) and their families; support is provided in person or by telehealth (including videolink), telephone, text, fax, or email. The support team consists of nurse practitioners, social workers, clinical pharmacists, and an infectious disease specialist. Services include connection to peer support, harm reduction services, and HIV testing; provision of temporary primary care; optimizing, accessing, and managing medications; optimizing nutrition; opioid agonist treatment; advocacy; and help in accessing community resources. Also provides clinical guidance and educational sessions to build community agencies and primary care providers' capacity to treat and support people living with HIV/HCV. Accepts referral from self or other including health care service providers. Hours are 8:30 am to 4:30 pm Monday to Friday.
