Home and Community Care - Fort St James

Offers health care and support services to adults with acute, chronic, palliative, or rehabilitative health care needs, and supports the transition from hospital release to home life; intake is required to assess eligibility.

Home Support provides assistance with medications, bathing, and dressing for clients who are ill or recovering from illness or surgery, have physical disabilities, require personal assistance or respite for a caregiver, or need end-of-life care. Nursing Services may include assessment, education, counselling, and medical and surgical care at home. Physiotherapy and Occupational Therapy referrals are for clients who have a physical disability, are recovering from surgery or hospital stay, or who need rehabilitation or therapy to stay at home. Provides assessment for access to Adult Day Services (day respite, health monitoring, therapeutic and recreational activities, and community bathing programs). Health Services for Community Living (HSCL) helps adults with developmental disabilities access non-emergency community health services such as nursing services, rehabilitation, care planning, dental hygiene, nutrition services, swallowing assessments, and family support; services delivered in partnership with Community Living BC. Does assessment for Choice in Supports for Independent Living (CSIL) which provides eligible home support clients with direct funds to hire, schedule, and supervise their own home support services. Also handles intake for publicly funded Assisted Living and Residential Care Facilities. Assisted Living provides semi-independent housing for older adults or persons with disabilities who require assistance with day-to-day activities such as medications, bathing, or life skills. Housing is intended for people who are able to make a range of decisions that allow them to live safely in a supportive, semi-independent environment. Residential Care Facilities provide housing for persons with complex health care needs who require 24-hour care due to physical disability or mental or behavioural conditions such as dementia. Services include nursing and personal care, medication administration, meals, laundry service, and social and recreational activities. Publicly funded bed fees are a monthly rate up to 80 percent of client’s after-tax income.

Other home health services available may include personal care, medication assistance, respite care, nutrition consultation, and health education. Also provides access to other specialty home health services. Serves the Bulkley-Nechako region including Binche, Fort St James, Tachie, and Yekooche. Office hours are 9 am to 12 noon and 1 pm to 4 pm Monday to Friday.
