Service Description

Regional health authorities provide clinical and support services in partnership with the family physician, for adults who require assistance for acute, chronic, rehabilitative, and end-of-life care needs. Community Nursing and Community Rehabilitation Services are provided at no cost, to support clients in home or community clinic settings following discharge from hospital, and to assist with management of acute or chronic health conditions or for end-of-life care. Home Support Services include personal care; assistance such as bathing, dressing, transferring to and from bed; and assistance with medications and therapeutic activities. Clients receive home support following professional assessment of their care needs and may be charged a daily rate based on after-tax income. Choice in Supports for Independent Living (CSIL) provides eligible home support clients with direct funds to manage their own home support services. The client (or client support group) assume full responsibility for management, coordination, and financial accountability. Adult Day Services offer organized health, social, and recreational activities in a group setting. Assisted Living provides housing, hospitality, and personal care services for clients able to direct their own care. Clients are charged a monthly fee of 70% of their after-tax income up to a maximum monthly rate. Residential Care Services are provided for clients with more complex needs who require 24/7 nursing supervision and care in a secure setting. Clients are charged a monthly rate of up to 80% of their after-tax income; clients who would experience serious financial hardship by paying their assessed client rate are eligible to apply for a temporary reduction of their client rate. Health Services for Community Living (HSCL) program provides specialized nursing and rehabilitation supports at the community level to eligible adults with developmental disabilities, including assessment, training, referral, and planning of supports and services for ongoing acute and complex health issues. Consultation is focused on the development of individualized health care plans, and specific training of individuals, caregivers, and families to manage the care needs of the client.
