Provides home nursing and home care services to community members with disabilities or Elders. Aim is to help individuals manage their own care, increase independence, and stay in their homes. Services include assessment, education, medical care, surgical care, counselling, medication management, wound care, support managing or preventing chronic disease, lunch preparation, and house cleaning.
