Teams provide low-barrier, culturally-safe, trauma-informed, wrap-around care by bringing services directly to people who are precariously housed, unsheltered, living in encampments, or living in shelters. Supports those who are experiencing mental health or substance use challenges, difficulties with their overall health, experiencing high rates of hospitalizations and emergency room visits, or not attached to a health care team. Also supports individuals' transitions to a new shelter or housing in collaboration with BC Housing and other social sector partners while supporting stability through continuity of care and quick response to escalating or destabilizing health, mental health, or substance use concerns. Regional network of multidisciplinary care providers (including nurses, social workers, health care workers, peer support workers, and Indigenous supports) works in collaboration with local partners to ensure integration of services in the community. Accepts referral from self or other.
