Intensive Case Management (ICM) - Abbotsford
CAUTION: RECORD IS INCOMPLETE
Provides intensive, community-based support for adults ages 19 and over who experience problematic substance use or chronic substance dependence, with or without mental illness, and who face complex challenges related to homelessness, unstable housing, poverty, trauma, or involvement with the justice system. Services are tailored to each person's goals and may include housing support, assistance accessing food, income and other basic needs, connections to medical, dental, specialist, psychiatric, and addiction medicine services, substance use counselling and treatment supports, life skills development, work skills rehabilitation, and referrals to community resources. The program uses a harm reduction, trauma-informed, culturally safe, and recovery-oriented approach and focuses on long-term engagement, trust building, and continuity of care. Available to adults who require more intensive support than traditional health and substance use services can provide. Not intended for individuals with severe mental illness without a substance use disorder, those whose primary concern is mental illness, or individuals with dementia. Self-referrals, referrals from community providers, and Fraser Health referrals are accepted. Referral forms must be submitted to the local intake team by fax. Serves Abbotsford and Mission. Hours are 8 am to 8 pm seven days a week.
