Provides community-based team care for individuals with moderate to severe substance use problems, concurrent disorders, and/or mental health issues, and those who may have complex needs related to community living, including housing and income. Support may include medical care, substance use counselling, access to treatment, life skill building, and referrals to community resources. Serves Victoria. Hours are 10 am to 4 pm Monday to Friday.
