Provides outreach support for people who experience severe substance abuse problems, chronic unmet medical needs, and multiple barriers for accessing services. Services are offered on a completely outreach basis and the goal is to stabilize clients and connect them to appropriate long-term services. Accepts referrals from VCH partners and St. Pauls' Hospital. Hours are 9:30 am to 3:30 pm Monday to Saturday.
