Service Description

Provides medical care and social support for newcomers to Canada (immigrants, government-assisted refugees, privately sponsored refugees, and refugee claimants) living in Burnaby and New Westminster during the first three years in Canada whose medical needs may be complicated by language, cultural, and knowledge barriers; priority is for pregnant women and children under age five. Services include prenatal care, vaccinations, health management, mental health support, and chronic disease management. Focus is on providing health management, education, and self-management support for those with multiple chronic conditions such as diabetes, and heart, lung, and renal disease. Staffed by a nurse practitioner who works collaboratively with physicians and community service providers. Service is in English with interpreters available in person or by phone. Referral is required from public health, social service, or settlement providers. Wheelchair accessible. Hours are from 8:30 am to 4:30 pm Monday to Friday; by appointment only.
