Provides outreach to youth ages 12 to 18 who are at risk of becoming less connected with their families and less involved in positive activities or choices. Outreach workers assist youth in making healthy decisions on a variety of issues and refer them to appropriate resources when needed. Workers also encourage the rebuilding of positive family connections. Services include one-to-one support and advocacy, as well as referral, and accompaniment to other community services.
