Connects adults ages 50 and over to local, non-clinical community services that will help them better manage their health and well-being. Support may involve physical and social activities, nutrition and food security programs, caregiver assistance, and access to community-based services. Self-referrals are welcome, as well as from physicians, healthcare providers, support workers, and other community members. Office hours 8:30 am to 4:30 pm Monday to Friday. Funded by Ministry of Health (MOH) and managed by United Way BC as a Social Prescribing program.
