Service Description
Connects adults ages 65 and over living at home in Tri-Cities or New Westminster with local non-clinical community activites and services that promote physical activity, nutrition, social connection, and independence. Helps older adults take control of their own health and become more engaged in their communities. Referral from a physician or other healthcare provider is required. Serves Coquitlam, New Westminster, Port Coquitlam, and Port Moody. Office hours are 9 am to 5 pm Monday to Friday. Funded by Ministry of Health (MOH) and managed by United Way BC.
