Social Prescribing for Senior Patients - Surrey

Connects Surrey adults ages 60 and over to local, non-clinical services that will help them better manage their health and well-being. Serves older adults who may be experiencing poor physical or emotional health, are frequent visitors to the emergency room or primary care, and/or are socially vulnerable, marginalized, or experiencing isolation. The social prescription addresses a person's social needs as part of their overall health and wellness plan, and allows for a comprehensive approach to multiple medical issues. Referral from physician or other health care practitioners required. Funded by Ministry of Health (MOH) and managed by United Way BC.
