Social Prescription Program

Connects older adults to local non-clinical community services that support participant health and wellness. Serves individuals experiencing poor physical and/or emotional health, marginalized individuals, socially vulnerable individuals, and those making frequent trips to ER or primary care. Works collaboratively with the participant's clinical team and refers to social programming in areas such as physical activity, skills and literacy, nutrition and food security, mental health support, medical transportation, and affordable housing assistance. Referral from a physician or a nurse practitioner is required. Funded by Ministry of Health (MOH) and managed by United Way BC.
