Connects adults ages 65 and over to local, non-clinical community services for support in the areas of physical activity, nutrition, and social engagement. Social prescription addresses a person's social needs as part of their overall health and wellness plan, and enables them to take control of their own health and engage in and contribute to their community. Referral required from a physician. Serves Chilliwack and Hope. Office hours are 9 am to 4:30 pm Monday to Friday. Funded by Ministry of Health (MOH) and managed by United Way BC.
