Connects Dawson Creek adults ages 65 and older with local, non-medical community services that can support them to be independent and actively engaged in their community. Serves individuals who may be socially isolated or at risk of frailty. Referral from physician or health care provider. Funded by Ministry of Health (MOH) and managed by United Way BC.
