Enables physicians and primary care clinics to quickly and easily refer their patients to community-based substance use services. Once a referral is made, the team will provide support and facilitate access to treatment for individuals by phone or in person as needed. Also accepts self referrals from patients. SUSAT South location is available 8:30 am to 6 pm Monday to Friday; SUSAT North and East locations are available 8:30 am to 6 pm Monday to Friday.
