Youth Concurrent Disorders Program
A specialized program that supports children, youth, and young adults ages 13 to 24 and their families who are navigating both mental health and substance use challenges. Offers compassionate, culturally safe, and trauma-informed care across the Fraser Health region through holistic screening, tailored treatment planning, individual and group therapy, family support, and systems navigation. The program also coordinates and connects clients to specialized Fraser Health services such as Early Psychosis Intervention, Eating Disorders, and Child and Youth Mental Health. Program emphasizes client-centered care and harm reduction approaches. Accepts referrals from self, family members, and health care professionals and community organizations through central intake line. Outreach-based service offered out of Abbotsford Foundry, Tri-Cities Foundry, Surrey Foundry, and Fraser Health Rural Primary Care Centre in Chilliwack. Serves Fraser Health region. Phone hours are 8:30 am to 4:30 pm Monday to Friday.
