Offers a variety of community services for residents of Hornby and Denman Islands including community education opportunities, equipment loan programs, seniors' initiatives, youth outreach, and child, youth, and family counselling. The Seniors Community Connector provides free social prescribing services to help seniors age 55 and older to reduce isolation and improve wellness. Works one-on-one with individuals to create personalized wellness plans and facilitates linking to community groups, social activities, and other programs. Program is inclusive of all backgrounds and supports those facing social frailty or barriers to community engagement; contact by phone or email at dee@hornbydenmanhealth.com. Hornby Hamper provides a food hamper during the holidays to families and individuals in need. Funded in part by Island Health and by Ministry of Children and Family Development (MCFD). Office hours are 9 am to 4 pm Monday to Friday. Nonprofit society, registered charity.
