Provides support services to youth ages 12 to 19 year old who have a lifelong developmental disability, Autism Spectrum Disorder, or a degenerative condition. Youth are supported in the home or community based on individual needs. Services include skills development to promote independence, health and well-being, and community inclusion. Referrals are through the Ministry of Children and Family Development (MCFD) At Home Program.
